Non-Emergency Medical Transportation Meridian provides options for routine transportation to and from your provider, behavioral health visits, pharmacies, durable medical equipment vendors and health departments. Non-emergency medical transportation is also covered for the Healthy Michigan Plan and pregnant members going to and from dental visits. You can also get paid back for gas when you, a family member or friend drives to and from office visits. Call Meridian transportation at 800-821-9369 for regularly scheduled appointments at least three calendar days before your appointment to talk about your options.
Have this information ready when you call:
• Your name, Medicaid ID number and date of birth
• The address and phone number of where you will be picked up
• The address and phone number of where you are going
• Your appointment date and time
• The name of your provider
• The reason for your appointment For members with any special needs (wheelchair accommodations, oxygen resources, etc.) will want to schedule those trips as early in advance as possible to match their needs with the appropriate vendor.

One pair of glasses Replacement of frames/lenses due to loss or breakage (if they cannot be repaired) is covered once each year for members age 21 and over and twice each year for members under age 21. “One year” is defined as 365 days from the date the first pair of eyeglasses (initial or subsequent) was ordered. More visits and eyeglasses are covered if medically necessary. Other medically necessary treatment for eye illness or injury is covered.
Call your provider right away if you injure your eye or have other eye problems. 24 Contact lenses need prior authorization and are covered only if medically necessary

Dental Services Meridian is contracted with a DentaQuest to provide your dental benefits for Healthy Michigan Plan members and pregnant members throughout pregnancy and postpartum (three months after the beneficiary’s pregnancy due date). Please see Member Benefits Book for additinal information.
